Dear Parents,

The Los Angeles County Department of Public Health is conducting a survey to better understand your access to nutritious food and community connections to improve your overall health and well-being. The survey asks questions to better understand where you shop for food, why you shop there, and your household food resources and challenges. The survey also asks questions about your connections to family and community. The survey is short and should only take you about 5-10 minutes to complete. We are asking our Mountain View families to complete the survey. 

**The survey is anonymous (it doesn’t ask for your name) and your answers to the survey are completely confidential.**
Survey results will help shape policies and services in our schools and the broader community.
